A scalp pigmentation treatment that is done with proper ink, will be somewhat permanent but not 100%. Over the course of about ten years you can expect it to fade to roughly half its original darkness. Even if you had gone completely gray by that time, you would still need a touch up in order for the top to be as dark as the sides. Going gray is not something that happens overnight, and even when it does happen it won't usually cause a significant change in the appearance of your hair when it is worn that short. Any color of hair is going to have a grayish appearance when shaved down with a razor. SMP treatments are also significantly blended into the area of existing hair, so it's not like you are going to see a distinct contrast between the two areas even if your hair did significantly change appearance.
What I can tell you is that SMP, when done right, is an excellent solution to hair loss but in most cases it is still not ideal. Almost everybody looks better with at least a small amount of visible hair growth on top, as opposed to just being completely shaved down. Every single time I ever posted pictures here, I was told by multiple people to "grow my hair out" until I explained that I had SMP and not real hair. That's certainly a testament to how real it looks, but at the same time it shows that many people aren't crazy about how I look right now either.
From what I have seen, the best hair restoration results usually come from doing SMP first and then getting a layer of FUE grafts on top so that you can actually grow some length. The SMP gives the illusion of high density which transplants aren't able to provide on their own in most cases. I've got a procedure scheduled with Dr. Doganay in Turkey, to put some grafts on top which will allow me to actually grow a bit of hair. His prices are very affordable and his results are excellent.
Are you in the United States? If so I could give you a few recommendations for which scalp micropigmentation providers to check out. There are only a few legit ones and ALOT of bad ones, and quite frankly it's hard to sift through all the bullshit and know the truth unless you're an insider like me.
Just saw your pics. Dude you are NOT ready for SMP. I say this as a SMP practitioner and I know what I'm talking about here. You haven't lost enough hair yet to be at the point where SMP will give you an overall benefit to your looks instead of a drawback. SMP is better than a fully bald head for sure...but if you're still at the point where you can grow out any kind of hair and still have it look half decent, you will look better by keeping the hair nearly 100% of the time. You would be the I-D-E-A-L candidate for a small transplant procedure in front, combined with a regimen of RU58841/Minox to prevent further hair loss. I'd estimate you would need no more than 2500-3000 grafts in front, which will cost you only about 5-6K (no joke) if you get it done in Turkey by that doctor I mentioned. I live in America but I was willing to pay roughly $800 for the flight to Turkey, because the transplant itself will cost me roughly 30% of what I would pay if I had it done by any equally-competent FUE surgeon in America or Canada. Travelling is usually worth the cost when it comes to cosmetic surgeries. If you got the procedure done with Dr. Rahal you could expect to pay at least 10K for the same results.
