I'd already told him that he's made enough threads about how ugly he is, and explained I'd ban him if he made another.

I'm not so sure about that. Identification is an important part of recovery as one needs to be reminded of where one started in order to appreciate the progress they're making. I for one have learnt a great deal about myself by reading posts from Mr. Ugly and Deltsbrah. It doesn't mean I'm gonna adopt their thoughts and actions, it's just helpful for me to understand where they're coming from so I can recognize when I start to spin the same record in my own head.

I understand the point about keeping the forum clean from stuff most people don't wanna read about and I'm not opposed to the ban, but at the same time it's a good source for self-reflection: why does it bother me so much when someone writes about how ugly he is? The fact I dislike something tells me more about myself than it does of the person writing/saying it.
